This is a simple way to give some journal feedback to students. Simply glue/staple it into their journal and check off what they've done! That way they can keep working towards including all elements of written work in their journal. This is a great tool for ongoing assessment.

I've included two versions of the student self-evaluation checklist - one with subject and one without. Subject means that they're writing about the proper topic (for use if you give students sentence prompts). I have also included a simple teacher assessment checklist. 

Please note this is very simple and is optimal for the younger grades who still need reminders to use capitals, put spaces and periods.
